If you've ever felt your throat tighten in an elevator filled with strong cologne, or developed a headache after someone spritzed perfume nearby, you're not alone. Fragrance sensitivity affects a significant portion of the population, yet it remains widely misunderstood and often dismissed as mere preference or oversensitivity.
What Is Fragrance Sensitivity?
Fragrance sensitivity encompasses several distinct types of reactions to scented products. The most common is chemical intolerance, where the body reacts to volatile organic compounds (VOCs) found in fragrances without involving the immune system. Another type of fragrance intolerance is due to fragrance allergy, which triggers an immune response and can cause skin reactions like contact dermatitis when fragrance ingredients touch the skin. Yet other reaction is due to having a neurological system that senses stimuli, including odor, much more strongly at much lower levels than the general population. Additionally, people with asthma, migraines, or other underlying conditions often find that fragrances exacerbate their symptoms, even if they wouldn't describe themselves as having a specific fragrance sensitivity.
Is this a case of people just being "too sensitive" or over-reacting? The answer is a resounding NO. Some people prefer to use the term "fragrance intolerance" because the word sensitivity could imply that fragrances are still tolerable, even if a person is more sensitive to them. Here I use both terms somewhat interchangeably. The reader should keep in mind that even if the word sensitivity is used, the reactions to fragrances are biologically driven and not a choice being made.
Multiple Chemical Sensitivity
Some people experience what's known as multiple chemical sensitivity (MCS), a condition where they react to various chemicals at levels that don't bother most people. Fragrances are among the most common chemicals that are intolerable for those with MCS.
Recent research has helped explain what's happening in the bodies of people with MCS. Scientists have found that in some people, certain sensory receptors in the nervous system become overly sensitive to chemicals after repeated exposures to pollutants and poor air quality [1].
Think of these receptors as alarm systems that detect chemicals. In people with MCS, these alarm systems become increasingly trigger-happy over time. The body also produces more of these receptors. The combination of higher levels of receptors that are also hyperactive means they start sounding the alarm at much lower levels of chemical exposure than would affect most people.
Because these particular receptors are found throughout the central nervous system and send signals to various parts of the body, the alarms they sound don't just affect one area. When these receptors are triggered, they can impact the respiratory system, digestive system, muscles, cardiovascular system, and more [1]. This explains why people with MCS can experience such a wide variety of symptoms even from very low levels of chemical exposure that most people wouldn't notice. Such symptoms include, but are not limited to, breathing difficulties, headaches, fatigue, cognitive problems, and digestive issues.
Essentially, repeated exposure to chemicals can rewire the nervous system's sensitivity, making the body react strongly and be intolerant to substances it would have previously tolerated. So, just because someone wasn't sensitive in the past doesn't mean that they are now making it up.
Other Causes of Fragrance Sensitivity or Intolerance
The biological mechanisms underlying fragrance sensitivity are complex and may vary between individuals. Many fragrance molecules are VOCs that can directly irritate the respiratory system's mucous membranes [2,3]. When inhaled, these compounds may trigger nerve receptors in the nose and airways, causing them to release neuropeptides that then cause inflammation and various symptoms.
For people with allergic reactions, specific ingredients in fragrances act as haptens, small molecules that bind to proteins in the skin and trigger an immune response [4]. Common culprits include ingredients like limonene, linalool, and various preservatives used in scented products [5]. Interestingly, these substances often become more allergenic after they oxidize upon exposure to air.
For some, fragrance sensitivity may be due to nervous system differences. Some researchers believe that in certain individuals, the nervous system processes sensory information from fragrances differently, leading to heightened responses [6]. This could explain why some people develop migraines or cognitive symptoms from fragrance exposure. Some people known to have neurological systems that differ from the average person, such as autistic people, may have a nervous system much more reactive to various stimuli, including fragrances.
How Common Is Fragrance Sensitivity?
Fragrance sensitivity is far more prevalent than many people realize. Studies suggest that approximately 30-35% of Americans report some degree of sensitivity to fragranced products. More specifically, research has found that around 12-15% of people experience health effects from exposure to fragrances, with symptoms ranging from mild to severe.
Certain populations are more affected. Women report fragrance sensitivity more often than men, though this may partly reflect differences in reporting rather than actual prevalence. People with asthma, allergies, or other respiratory conditions are also significantly more likely to have fragrance sensitivity.
Recognizing the Symptoms
The symptoms of fragrance sensitivity or intolerance vary widely from person to person, but commonly include respiratory issues like difficulty breathing, coughing, wheezing, or a tight feeling in the chest. Headaches and migraines are extremely common, as are nasal symptoms like congestion, runny nose, or sneezing.
Many people experience neurological symptoms such as difficulty concentrating, dizziness, or feeling mentally foggy. Skin reactions can occur on contact, ranging from redness and itching to full-blown dermatitis. Some people report nausea, eye irritation, or even fatigue after fragrance exposure. For those with asthma or migraines, fragrances can trigger full episodes of their underlying condition. In fact, research has found that 70% of migraine patients report headaches triggered by odors, with perfumes being the most common trigger [7].
The severity of these symptoms shouldn't be underestimated. For some people, exposure to strong fragrances can mean leaving work early, missing social events, or experiencing days of discomfort from a brief encounter with scented products.
A Call for Greater Understanding
Given how many people are negatively affected by fragrances, we need to cultivate more awareness and consideration in our communities. This starts with recognizing that fragrance sensitivity—and particularly multiple chemical sensitivity—is a real, complex condition involving both neurological and physiological factors, not a personal preference, psychological issue, or an attempt to be difficult.
The impacts on people's lives are profound and far-reaching. Research shows that fragrance and chemical sensitivities can significantly impair people's ability to work, attend school, maintain social relationships, and access healthcare services [8]. For some, exposure to fragrances in public spaces means being excluded from participating in everyday activities that others take for granted. Beyond the physical symptoms, people with these sensitivities often face stigma and discrimination, with their experiences dismissed or their reactions characterized as overreactions or imagined problems.
These misconceptions and the beliefs that chemical intolerances are "all in your head" or that people are simply being overly dramatic create real barriers to care and support [8]. When we fail to recognize the scientific validity of these conditions and their real-life impacts, we perpetuate a divide between those affected and those who aren't. That, in turn, makes it harder for people with sensitivity to advocate for their needs or receive appropriate accommodations.
It's important to understand that fragrances negatively impacting some people aren't limited to just perfumes and colognes. People with fragrance sensitivity often react to a wide range of scented products, including air fresheners, scented cleaning supplies, and laundry products like detergents, fabric softeners, and dryer sheets [9]. In fact, scented laundry products can be particularly problematic because the fragrances linger on clothing and linens for extended periods, and fragrances from dryer vents can affect neighbors and passersby. Even seemingly innocuous products like scented hand soaps in public restrooms or cleaning products used in shared spaces can trigger significant reactions.
Reducing the use of fragranced products in workplaces, schools, healthcare facilities, and public transportation can make these environments more accessible to everyone. This doesn't mean eliminating personal care products entirely, but rather choosing unscented or fragrance-free options when possible, especially for products like hand soap, cleaning supplies, and air fresheners in common areas.
Many workplaces and institutions have begun implementing fragrance-free policies, recognizing that accommodating fragrance sensitivity is similar to other accessibility considerations. These policies aren't about catering to personal preferences; they're about removing systemic barriers that prevent people from fully participating in work, education, and community life.
On a personal level, we can all be more mindful. Before applying heavy perfume or cologne, consider the enclosed spaces you'll be in and the people around you. When doing laundry, consider using unscented detergents and skipping the fabric softener, especially in multi-unit buildings where dryer vent emissions affect others. Choose unscented products when you have a choice. And if someone mentions they have fragrance sensitivity, believe them and work with them to find solutions rather than dismissing their concerns.
Fragrance sensitivity isn't about being overly particular or disliking what to others are pleasant scents. It's about real biological reactions that can significantly impact quality of life. By choosing to use fewer fragranced products and being understanding when others express sensitivity, we create more inclusive environments where everyone can breathe a little easier, literally. Greater societal awareness and policy interventions are needed to address the barriers faced by people living with these sensitivities, ensuring they can participate fully in all aspects of life.
